We are an appointed committee responsible for overseeing all research projects involving human participants.

Federal Wide Assurance (FWA)
The FWA is a contract or agreement that establishes standards for human participants research as approved by the (OHRP). It is the policy of the 91大神 IRB to uphold its Assurance as filed with the OHRP. See more.
Announcement
The following changes have been made to the IRB Initial Submission application. Please read all instructions when completing new protocol submissions as some instructions may have changed.
- PDF requirement for attachments – All attached documents must now be submitted as PDF files, since Cayuse can only apply IRB stamps to PDF documents.
- Use of stamped documents – Investigators must use only the IRB-stamped versions of consent forms and recruitment materials once approved.
- Clarified instructions throughout the application – Added more detailed descriptions and instructions across sections, including links to templates and required elements, to reduce common submission errors.
- Data storage section restructured – Replaced the open-ended data storage question with checkboxes for specific storage types (on-campus storage, 91大神-issued device, 91大神-approved cloud service, or other), each with corresponding guidance on required details (what is stored, how it’s secured, retention duration).

Institutional (FERPA) Data
Institutional data is any data that is utilized by the university for non-research purposes. Even if the data does not fall under the FERPA definition it is still subject to permission to utilize institutional data.
The Family Educational Rights and Privacy Act (FERPA) is a federal law that protects the privacy of student information and mandates that educational institutions uphold strict confidentiality standards.

REVIEW AND SUBMISSION

FILL OUT APPLICATION
Review takes place in stages as noted on the Cayuse portal; researchers can be asked to add additional information at any stage and all requested information must be supplied before final approval can be granted.
To request for a new user to be added to Cayuse, fill out the (Dynamic Form).
