| Dual Enrollment Consent form | Complete the form here. |
| Consortium Agreement (if applicable) | If student is taking Dual Enrollment courses at more than one college/university in same semester, contact the Dual Enrollment office for additional paperwork. |
| Submit College Transcripts (if applicable) | Submit official transcripts of previous dual enrollment coursework if you have taken classes at an institution other than 91大神. Transcripts can be sent to the 91大神 Admissions Office. |
| Submit Fee Discount Forms (if applicable) | Complete a fee discount form – for dependents of 91大神 or State of Tennessee employees. |
